- 1(1) 2A voter information card shall be furnished by the supervisor to all registered voters residing in the supervisor’s county. The card must contain:25(a) 26Voter’s registration number.29(b) 30Date of registration.33(c) 34Full name.36(d) 37Party affiliation.39(e) 40Date of birth.43(f) 44Address of legal residence.48(g) 49Precinct number.51(h) 52Polling place address.55(i) 56Name of supervisor and contact information of supervisor.64(j) 65Other information deemed necessary by the supervisor.72(2) 73A voter may receive a replacement voter information card by providing a signed, written request for a replacement card to a voter registration official. Upon verification of registration, the supervisor shall issue the voter a duplicate card without charge.112(3) 113In the case of a change of name, address of legal residence, polling place address, or party affiliation, the supervisor shall issue the voter a new voter information card.
