SECTION 636.057. Fees.

Latest version.

1Every prepaid limited health service organization subject to this act must pay to the office the following fees:
19(1) 20For filing an application for a certificate of authority or amendment thereto: $500.
33(2) 34For filing each annual report: $200.
40(3) 41For each renewal of certificate of authority: $500.
