SECTION 607.0121. Forms.

1(1) 2The department may prescribe and furnish on request forms for:
12(a) 13An application for certificate of status,
19(b) 20A foreign corporation’s application for certificate of authority to transact business in the state,
34(c) 35A foreign corporation’s notice of withdrawal of certificate of authority, and
46(d) 47The annual report, for which the department may prescribe the use of the uniform business report, pursuant to s. 66606.0667.
68(2) 69If the department so requires, the use of these forms shall be mandatory.
82(3) 83The department may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter, but their use is not mandatory.
